We offer coordinated care management for both medical and behavioral health care, because most people don’t see their physical health and their mental health as separate – and neither do we.
Our office has a care management agreement with Amy Foster, LPA and her associates at Fostering Minds, and are conveniently located in the same building.
Effective Jan. 1, 2015, federal regulations now enable Medicare to pay for chronic care management services. Chronic conditions are ongoing medical problems that must be managed effectively in a partnership between the health care team and the patient to maintain the best possible health. Examples include diabetes, high blood pressure, heart disease, depression, and others.
Your physician and primary care team will carefully monitor and provide comprehensive care for your chronic health conditions in a systematic way to supplement regular office visit care.
Dr. James McGrath is now accepting new patients, both pediatric and adults. Please see the links below to download and print our new patient application and records release. If you are a former patient of Dr. McGrath's or Yadkin Valley Community Physician's, you will need to also download and fill out the medical records release form from the county, so that we may obtain your records from them as well.
If you live in the Yadkin Valley area, you can stop by our office for more information.
We now offer Chronic Care Management at our practice.
